Appointment Request Form
Used by property owners, representatives, or filing professionals to request a DOB appointment for matters such as Stop Work Order rescission, Vacate Order rescission, corrective action reinspection, complaint inspection, or violation dismissal.

Gas Piping System Periodic Inspection Certification (GPS2)
Used by a licensed master plumber or authorized representative to certify the results of a required gas piping inspection for buildings covered by the NYC gas piping inspection rules.

Identifying Information Complaint Form
Used by a person who believes DOB or its contractor collected, kept, or shared identifying information in violation of the NYC Identifying Information Law.

Certificate of Correction (AEU2)
Used by an owner or authorized representative to show that a DOB violation condition was corrected, although DOB states that AEU2 is no longer generally required as a separate form in the current correction process.
